Types of Services
Supported Employment Service
This service has a proven track record of successfully placing individuals with disabilities in a variety of community jobs. The continued support provided have made it possible for individuals not only to secure employment, but also to maintain that employment.
Vocational Program
This service provides training of individuals while on a work crew or an enclave. Supervisors work with the individuals to develop, maintain, or increase job skills and opportunities. For those individuals who may not be ready for a work crew/enclave, Port City Enterprises can provide Pre-Vocational Training or Habilitation Training.
Community Living Program
This service provides Supervised Independent Living, Personal Care Attendant, and/or Respite Care Services to individuals or families through the Medicaid Waiver Program or family support. This provides opportunities for individuals to increase their independence in such areas as self-help and community integration.
Each Medicaid recipient of targeted or waiver case management has the freedom to choose their providers. The recipient's choice of case management or waiver service(s) providers does not affect their management services.
Recipients of Medicaid Case Management Services may report any problems with their case management services or waiver services to the Bureau of Community Supports and Services helpline manager at 1-800-660-0488.
